All Persons 18 Years of Age and Older are Eligible to Receive the Vaccine in Florida

As of Monday, April 5, all Florida residents shall be eligible to receive any COVID-19 vaccine as prescribed by the Food and Drug Administration.
• PERSONS AGES 16-17: the Pfizer vaccine is authorized for persons age 16 and up. The Moderna and Janssen (Johnson and Johnson) vaccines are authorized for persons age 18 and up.
• All individuals under the age of 18 receiving the vaccine must be accompanied by a guardian and complete the COVID-19 vaccine screening and consent form. To download a copy of the form, click here.

Appointments are required to receive a COVID-19 vaccine through USF Health, and our team is currently scheduling eligible patients. To schedule your vaccine appointment please log into MyChart.
